Care Manager Program

Our program will focus on providing Care Managers and exercise therapy
Although this is a psychological field, we believe only through the integration of the physical body, mind and spiritual being, people suffering depression are able to achieve better results of healing.  According to individual’s particular circumstances, Alive Coaching Foundation will customize a program which contains exercise, companionship, listening, spiritual and recreational activities.  Alive Coaching Foundation provides a "care manager" to integrate and coordinate various activities.   Our program needs many aides to implement its goals.   We strongly believe exercise can benefit participants physically and spiritually.  It is a good approach in our opinion.  For instance, Alive Coaching Foundation can send personal trainers to assist family members of people suffering from depression to encourage participants to engage in various activities outside their home, change lazy habits, increase positive energy, and get out of their suffering to lead a normal life. .


1. Assistance Timeline and Process:
Intense Phase: 6-12 weeks; Intermediate Phase: 4 -9 months; Recovery Phase: One year 

2. Assistance that can be provided by Alive Coaching Foundation:
a. Assessment and strategy development - assess and gather information about participants and their family situations.  Establish an interactive relationship to jointly develop assistance plans.
b. Medical doctor - referring, appointment scheduling, timely reminding and transportation to see doctors.
c.  Family members - require two or more relatives and friends take turns to work with the participant.  Those care givers should learn how to adapt their lives.
d. Companionship -  friends and relatives take turns to accompany the participant.  The participant is not left alone, sometimes forgotten .
e. Rehabilitation - to accompany the participant for exercise or go out for a walk or shopping .
f. Psychological consultation - referring, appointment scheduling, timely reminding and transportation for psychological consultation.
g. Establish exercise habits - based on individual circumstances, make exercise become a part of daily life.
h. Recreational activities - according to each participant’s personal interest, engage in various recreational activities including the arts, music, gardening etc.
i. Return to society – help the participant to gradually recover to interact with others, connect with outside world, to return to full time employment or return to school

3. Funds are for:

a. compensation to family members who quit jobs to care for participants
b. payment of recreational activities or athletic club membership
c. compensation to aides: care manager, transportation personnel, personal trainer, psychological consultation, various course tutors etc.
